SUMMARY OF REQUIREMENTS FOR ALL GRADES BEFORE SCHOOL ENTRY-NO PHYSICAL/NO SHOTS=NO SCHOOL!:
- Children entering PRE K OR EARLY CHILDHOOD PROGRAMS, KINDERGARTEN and SIXTH GRADE must have an updated physical, completed within one year, and required immunizations turned in BEFORE the first day of school.
- EYE EXAMS are required for all children entering KINDERGARTEN and for all students entering an Illinois school for the first time. Completed IDPH
- DENTAL EXAMS are required for children in KINDERGARTEN, SECOND AND SIXTH GRADES. Completed IDPH dental forms should be submitted by May 15th.

Here is a brief summary of grade level health requirements & requirements for participating in a Sport and/or Extracurricular Activities:
PRESCHOOL AND EARLY CHILDHOOD PROGRAMS:
- Physical examination completed within the last year, on an IDPH form, with proof of required immunizations.
- Please be sure your health care provider completes diabetes and lead screening as required!
- Proof of physical exam, diabetes and lead screening and required immunizations must be submitted before the first day of school.
KINDERGARTEN:
- Physical examination completed within the last year, on an IDPH form, with proof of required immunizations.
- Please be sure your health care provider completes diabetes and lead screening as required!
- Proof of physical exam, diabetes and lead screening and required immunizations must be submitted before the first day of school.
- Eye examination completed by an eye doctor, on an IDPH form and submitted by October 15th.
- Dental examination completed by a dentist, on an IDPH form and submitted by May 15th.
SECOND GRADE:
- Dental examination completed by a dentist, on an IDPH form and submitted by May 15th.
SIXTH GRADE:
- Physical examination completed within the last year on an IDPH form,
- Proof of all required immunizations, including the Tdap and varicella (chickenpox) vaccine all submitted before the first day of school.
- Dental examination completed by a dentist, on an IDPH form must be submitted by May 15th.
STUDENTS NEW TO THE DISTRICT:**
- Physical examination completed within the last year, on an IDPH form, with proof of required immunizations. This must be submitted before the first day of school. If the student is transferring from an Illinois school, the physical still must be submitted before the first day of school along with proof of immunizations.
- Grade level requirements as mandated above (dental and/or vision)
STUDENTS WHO ARE COMING FROM OUT OF STATE:
- Physical examination completed with in the last year, on an IDPH (Illinois Department of Public Health) form with immunizations per Illinois school code.
- Eye examination, completed by an eye doctor on an IDPH form, is also required by October 15th.
Sports Participants & Extracurricular Participants Requirements
Roscoe Middle School SPORTS PARTICIPANTS:
- CURRENT sports physical on file submitted BEFORE TRYOUTS.
Roscoe Middle School SPORTS, SCIENCE OLYMPIAD, QUIZ BOWL, JrNHS, STUDENT COUNCIL, SCHOOL PLAY PARTICIPANTS:
- Proof of health insurance, Medicaid or Allkids & signed parent permission must be submitted before trying out for a sport or participation in any extracurricular club or sport at Roscoe Middle School.
